- PTA Waiver – REQUIRED, one per FAMILY-- This is the ONLY one that needs to be printed and returned.
- CAC Email List (for parents of children with IEPs and 504's at AV)

- Middle School Specific Spirit Wear Paper Order Form (only way to order)
